Step 1: Tell us about yourself and what kind of visit you need

Important Information about In Home Visits

1. Please be aware that there is a $70 travel/convenience fee for each home visit. This fee is NOT billed to or paid by insurance.


2. BCBS of Texas plans often assign the entire billed amount to patient responsibility for home visits, so we recommend choosing office or virtual visits.

Important Information about Telehealth Visits for United Healthcare

Please be aware that due to issues with billing, we do not accept United Healthcare for telehealth services for lactation. If you have a United Healthcare plan, you will be self-pay for any virtual/telehealth visits.

Insurance Coverage for Lactation Services

If the adult patient is not expecting, lactating or trying to induce lactation, many insurers will not cover the visit so you may be responsible for paying for your visit out of pocket.

Important Information about Insurance and Self Pay

By choosing to not provide us with your insurance information, you are agreeing to self-pay for your visit. Please see our Cash Pay Price List for information on our Timely Payment Cash Pay rates.

We do offer a payment plan if needed, and can accept HSA or FSA cards for payment.

Please make sure to fill in all the information, as missing information will delay your insurance verification.

Important Information about BCBS Plans

We are in network or can accept or bill many BCBS plans.

1. PPO plans are usually accepted.
2. We are in network with BCBS UT Health Select
3. We are in network with BCBS Federal Plans
4. HMO PLANS: Due to billing and referral issues, we are NOT ACCEPTING HMO Plans from BCBS. If your insurance card displays the letters HMO, HME or BVA, you have an HMO plan
5. OUT OF STATE PLANS: We can accept many out of state BCBS plans.
6. COST SHARING: We often see billing problems and cost sharing with the following plans: TRS Activecare Plans, Blue Essentials and Blue Advantage. If you have one of these plans, you will likely have out of pocket costs for your visits.
7. HOME VISITS: BCBS of Texas plans often assign the entire billed amount to patient responsibility for home visits, so we recommend choosing office or virtual visits.

8. If cost-sharing is applied, we will do our best to have it removed. This may include filing a complaint with the Texas Department of Insurance or the Department of Labor on your behalf. If our attempts to appeal are denied, we will have charge you for the amount your insurance applies to patient responsiblity.

For other United Plans that use the United Healthcare network, we recommend that you contact your insurance provider to determine if lactation benefits are provided on your plan. Examples of these other insurances that use the United Healthcare network include the following: Allsavers, Surest, Bind Benefits, UHSS, UMR, United Healthcare Shared Services, United Healthplans Inc. and US Networks UHC Global.

HMO PLANS: If you have an HMO plan, you will need a referral for both you and your baby to have your insurance cover our visits.

TELEHEALTH: Please be aware that due to issues with billing, we do not accept United Healthcare for telehealth services for lactation. If you have a United Healthcare plan, you will be self-pay for any virtual/telehealth visits.

COST SHARING: If cost-sharing is applied, we will do our best to have it removed. This may include filing a complaint with the Texas Department of Insurance or the Department of Labor on your behalf. If our attempts to appeal are denied, we will have charge you for the amount your insurance applies to patient responsiblity.

Important Information about KelseyCare HMO Plans

We are in network with KelseyCare, but a referral is required.

In order to see one of our lactation consultants, you will require a referral for BOTH you AND for your baby. Please contact your OB/GYN and your Pediatrician and request that they submit a referral request for each of you to us.

Important Information about Boon Chapman Plans

We are in network with Aetna and can usually bill most Boon-Chapman Plans.

There are a few things we need you to aware of regarding Boon Chapman Plans.

• Some Boon Chapman plans do not cover lactation services, please contact Boon Chapman to see if your plan has lactation benefits.
• Boon Chapman will often apply cost-sharing to our claims.
• If they apply cost-sharing, we will make one appeal. If that appeal is denied, we will charge you for the fees applied.

Important Information about Meritian Plans

We are in network with Aetna and can bill Meritian Plans.

There are a few things we need you to aware of regarding Meritian Plans.

• Meritian covers a maximum of 6 lactation visits.
• If you have taken any "free" or "insurance-covered" prenatal classes through Aeroflow, you may have used up some of your visits..
• After the 6th visit, Meritian will usually apply your deductible to your visits.
• Meritian will often sometimes apply cost-sharing to our claims.
• If they apply cost-sharing, we will make one appeal. If that appeal is denied, we will charge you for the fees applied.

Important Information about Aetna Plans

We are in network with Aetna and can bill most Aetna Plans.

There are a few things we need you to aware of regarding Aenta

• Fully-funded Aetna plans cover a maximum of 6 lactation visits without cost-sharing.
• Self-funded Aetna plans (example: Exxon Mobile plans) usually apply a specialist copay to all visits.
• If you have taken any "free" or "insurance-covered" prenatal classes through Aeroflow, you may have used up some of your lactation benefits and may see cost-sharing applied before your 6th visit.
• After the 6th visit, Aetna will usually still cover visits; however, they will apply cost-sharing (co-insurance or deductible) to your visits.

• If cost-sharing is applied, we will do our best to have it removed. This may include filing a complaint with the Texas Department of Insurance or the Department of Labor on your behalf. If our attempts to appeal are denied, we will have charge you for the amount your insurance applies to patient responsiblity.

Important Information about Marpai Plans

We are in network with Aetna and can bill Marpai Plans that use the Aetna Network.

• Marpai may apply cost-sharing to our claims.
• If cost-sharing is applied, we will make one appeal. If that appeal is denied, we will charge you for the amount your insurance applies to patient responsiblity.

Aetna Hermann Memorial ACO Plans

If you have Hermann Memorial Aetna ACO, you WILL need to get a GAP coverage exception prior to your first visit if you want us to bill your insurance for your visits.

Important Information About Cigna Plans

• We can accept most Cigna PPO plans through our partnership with The Lactation Network
• We are UNABLE to bill the Cigna Allegiance Methodist plan

Important Insurance Information

Unfortunately, we do not accept the following insurance plans, even with a GAP exception. You will need to pay out of pocket for your visit.

• Ambetter
• Community Health Choice
• Hermann Memorial (non Aetna Plans)
• ALL Medicaid plans
• KelseyCare United Healthcare Charter Plan
• Methodist HMO plans
• Molina
• Texas Star Plans
• TRICARE West

You will need to self-pay for your visit and submit a superbill to your insurance provider for reimbursement. For more information, please see our document on how to request reimbursement from your insurance company.

We do offer a payment plan if needed, and can accept HSA or FSA cards for payment.

For families with financial need, we also have a grant/scholarship program.

Important Information about BCBS HMO Plans

Due to problems with HMO referrals and billing for BCBS we are NOT ACCEPTING new BCBS HMO Clients at this time.

If you would like to use your BCBS HMO benefits, you can contact Liquid Gold Lactation as they are currently taking BCBS HMO clients
