Billing Policy

We will submit a claim to Curative for your lactation services. We will attempt to pre-verify your benefits, but we do not guarantee coverage or benefits. 

You are responsible for knowing and confirming your own lactation benefits, and confirming that we are an in network provider on your specific plan and if there are any restrictions or limitations on your benefits.

If your policy applies a copay, coinsurance, or deductible, you will be responsible for paying these charges. The copay will be billed within 24 hours of service. We are legally obligated to bill you for these charges. 

Our insurance billing for both mom and baby reflects the personalized care and attention provided during the appointment. Since both mom and baby are assessed as patients, claims are submitted for each to ensure all aspects of care are properly covered.

Please be advised that Curative does not allow separate billing for both mother and baby on the same date of service. There will be up to a $60 non-covered infant fee for claims under the same policy as mom.
